What's that entry in the Death Register?

understandinguncertainty.org was produced by the Winton programme for the public understanding of risk based in the Statistical Laboratory in the University of Cambridge. The aim was to help improve the way that uncertainty and risk are discussed in society, and show how probability and statistics can be both useful and entertaining.
Many of the animations were produced using Flash and will no longer work.
My Father died back in August 1998, and his name was Stanley Round. His entry in the Death Register necessarily would be S Round.
Several days after he died (in hospital), I took my Mother to the appropriate office in Lincoln, to register the death, with the appropriate documentation.
Apparently, at that time, a single book was used to register deaths, but as there were several offices in the building that were used to register deaths, the book was moved between the offices, as and when the necessary details were needed to be added.
My Mother and I were called into the office, to a man sitting at a desk, and he had the death register already open in front of himself. He asked who we were, our business, and then asked for our documentation. He then sat in silence for some minutes in front of us, and went through our documentation visually several times, becoming increasingly flustered, and concerned.
Then, very carefully, and formally, he carefully asked us if it was possible that another member of the family had registered the death prior to us coming to the office?
Not knowing what was going on at all, I then carefully explained that it was impossible that anyone had previously registered my Fathers death.
He now became even more upset and flustered, and checked the details we had provided yet again, and then gradually became calmer, and sat back, and in silence, passed the Death Register to me and my Mother, and then said, look at that!
Wow! The last entry in the Death register was a S. Round!! Not Stanley, but (I think) a Sheila Round, who had died a few days earlier.
So in that Death Register, there are two consecutive entries of the deaths of two S. Rounds!!
What's the chances of that??

I actually, rather naughtily, (and I still have my notes somewhere) memorised the name and address of this S. Round, and sent a letter of condolence to that address, explaining why I was writing, but sadly received no reply! A shame!
